Health Form: This can be downloaded above. You can type directly on the form, even in the signature section. Once completed, save it as ("camper last name-first name" Health Form) and return via email to our office.
A copy of your child's immunization history must also be emailed or faxed to our office to complete the health form, if you have not filled in the information in that section yourself. Both the health form and immunization history must be sent in for this form to be considered complete. You will continue to receive emails if one or both are missing.
EpiPen/Medication Form: This can be downloaded above and you can type directly on the form, even in the signature section. However, the bottom section must be completed by the camper's physician. Once completed by parent/guardian and the physician, please email or fax it to our office. If emailing, please send the file as ("camper last name-first name" EpiPen Medication Form). “Epi-Pen” or Medication MUST also be handed in at Monday arrival in a labeled zip lock bag with a copy of this form.
