Disclaimer Form: *required* Submitted - and electronically signed - through your CampInTouch account.
Health Form: *required* Downloadable form that must be completed and signed by parent/guardian, and then can be uploaded to your CampInTouch account OR emailed to the camp office.
Immunizations: *required* Copy of your child's immunization history must be uploaded to your CampInTouch account OR emailed to the camp office to complete the health form requirements.
'Epi-Pen' / Medication Standing Order Form: Downloadable form - only required for campers attending camp with an 'Epi-Pen' or Medications - that must be completed and signed by parent/guardian, AND child's physician, and then can be uploaded to your CampInTouch account OR emailed to the camp office.
Transportation Form: This form is only for families who have added the optional transportation services, and can be submitted - and electronically signed - through your CampInTouch account.
